Sport Club Forms
Below is a list of all the forms that need to be turned into the Sport Club Office (SCO). You can download these forms and turn them in to our office, or email them to the Sport Club Coordinator. Please pay close attention to the timeline in which these forms need to be turned in.

| Van Requests | Any club that wants to rent a van for legitimate club use |

| Roster | The SCO uses this form to check eligibility as well as obtain
excuse letters for club events. Please type the name and 901 numbers of your team using this form |

Please also note that your club is responsible for turning in a current copy of your insurance policy and making sure that two members of your team have current CPR and First Aid certifications. The SCO will schedule two CPR/First Aid trainings during the year if your club needs to meet this requirement. Please turn in a current copy of the CPR/First Aid cards to the SCO.
