Registration Forms

 

 

The below forms, the exception being the "Campus Recreation Waiver" are write-able, so please fill out the required information, print, and submit to the CRC Member Service Desk...along with your respective payment.

The "Credit Card Payment" form is not required to be filled out if you are bringing your own payment to the CRC. It should be used ONLY if you are bringing forms (and payment) for someone other than yourself.
