Photography and Videography Request

Please describe where you would like to film or take photos in the facility. Additionally, please provide a short description of the purpose of this project.
Please note that videography and photography requests take approximately one month to finalize. Therefore, we are unable to accommodate requests with a turnaround time of less than one month.

GIT FIT Customized Program Request

Everyone can benefit from the restorative touch of massage, but we understand that, sometimes, massage needs to be brought to your space! By offering massage therapy at your next function or as a “well-being break” for your department, your group will reap the rewards of reduced stress, increased appreciation for your organization, and increased morale. Let the healing touch of massage add that something extra to your next program, event, or well-being break. Please note: this form is for making a request only. Completion of the form does not guarantee approval of your request or confirmation of therapists. The Requestor will be notified by email or telephone of the approval / denial of the request once it is reviewed. Anyone can request our customized on-site therapy program; however, priority is given to students, faculty, and organizations approved by Georgia Tech. Fees vary based on university affiliation and direct costs needed to support the event/activity. Charges for facilities, set-up, clean-up, staffing and/or equipment may apply to each event and must be understood and agreed upon prior to the reservation confirmation being finalized. All requests less than 14 days in advance will be evaluated on a case by case basis. Events consisting of any guests not affiliated to Georgia Tech (students, faculty, approved organizations, etc.) must be declared in advance. Note: If a request includes external participants, you MUST check External Group/Guests. Failure to acknowledge/indicate this can result in the cancellation of your reservation. Requests are received and approved on a first come, first serve basis after Campus Recreation programming needs are filled. Event requests for non-university related individuals must adhere to additional requirements and procedures per university regulations. Due to safety and liability concerns, additional insurance and increased supervision may apply. The need for chaperones will be required (due to age restrictions) for special events in Campus Recreation Facilities involving minors and/or anticipated guests.

Personal Training Request

General Information
Upon completion, you will be contacted by a GIT FIT staff member within three business days. We do our best to match you with a trainer as soon as possible. Most clients are matched with a trainer within a week or two after all paperwork has been submitted. However, please understand that many of our trainers are full-time students with limited availability. If you have any questions please email stephanie.belcher@crc.gatech.edu or call (404) 385-7529.
Please list a minimum of three days of the week with times you're available. (Example: Mon/Wed, 3 p.m. - 6 p.m. or Tues/Thurs, 1 p.m. - 5 p.m.)

Refund Request Form

General Information
Once submitted, please allow five to ten business days for review. After review, you will be contacted regarding the status of your request. If the request is approved, a refund will be processed via the original payment method. Provide as much information as possible do not resubmit previously submitted requests. Issued credit for future use cannot be refunded. Denied refunds may not be appealed. All approved refunds must be claimed within 30 days of notification. After 30 days, unclaimed refunds are no longer valid.
